Consumer Takaful Contract 

Pursuant to Section 5 of Schedule 9 of the Islamic Financial Services Act 2013, you are obliged to answer all questions required in this Proposal Form fully and accurately and disclose that you know to be relevant to our decisions in accepting the risk and determining the rates and terms to be applied, otherwise it will result in voidance of your contract, refusal of claims or changing of 

the terms. This duty of disclosure shall continue until the time the contract is entered into, varied or renewed. You are obliged to take reasonable care not to make any misrepresentation in answering the questions and in making the disclosure.

Non-Consumer Takaful Contract
